DISABILITY SERVICES
Services are available to help people with disabilities live in their communities as independently as possible. Assessments are completed to determine needs, preferences, and eligibility for an individual. Case managers coordinate services based on the individual’s service plan from the assessment. To request an assessment or get more information, please call 218-634-2642.
Home and community-based waivers may be available to fund services for those who qualify.Â

Consumer Directed Community Supports (CDCS)Â
Consumer Directed Community Supports (CDCS) is a service option under each of the waivers that gives the flexibility and responsibility to tailor services to an individual’s unique needs. This program requires coordination with a fiscal support entity and often includes assistance from a support planner. Contact a county case manager at 218-634-2642 to learn more.Â
Community First Services and Supports (CFSS)Â
Community First Services and Supports (CFSS) is a flexible option for those not seeking waiver services to still get goods and services, similar to a PCA, to meet their disability needs. Click here for a brochure.

The Social Services Office is open Monday through Friday from 8:00 a.m. until 4:30 p.m. To file a child protection report after hours, call Law Enforcement at 218-634-1143 or our after hours children’s emergency response number at 218-395-0177. We accept collect calls before 4:30 pm.
