Services are available to help people with disabilities live in their communities as independently as possible. Assessments are completed to determine needs, preferences, and eligibility for an individual. Case managers coordinate services based on the individual’s service plan from the assessment. To request an assessment or get more information, please call 218-634-2642.
Home and community-based waivers may be available to fund services for those who qualify.
Community Access for Disability Inclusion Waiver (CADI)
Developmental Disabilities Waiver (DD)
Community Alternative Care Waiver (CAC)
Consumer Directed Community Supports (CDCS)
Consumer Directed Community Supports (CDCS) is a service option under each of the waivers that gives the flexibility and responsibility to tailor services to an individual’s unique needs. This program requires coordination with a fiscal support entity and often includes assistance from a support planner. Contact a county case manager at 218-634-2642 to learn more.
